Admission Form :

The candidate has to apply on the prescribed Performa included in the prospectus in his or her own handwriting and candidates also download application form here.
Documents requried :

1.  Mark sheets: Two sets of photocopies of mark–sheet duly attested, qualifying/ last attended examination (Higher Secondary (10 + 2), Graduation, Post Graduation).
2.  Recent passport size photograph, duly attested should be posted on the application form and 2 photographs are also enclosed.
3.  All admission are provisional, subject to verification of the required document & eligibility conditions.
